ABSTRACT

Casos de fraturas dentárias oblíquas muitas vezes possuem um prognóstico desfavorável, geralmente com indicação da extração do elemento dental. O presente artigo visou relatar o tratamento do caso clínico de um paciente do sexo masculino, 12 anos, que compareceu à Unidade de Saúde Bucal do Hospital Universitário de Brasília com encaminhamento de extração do dente 12, apresentando fratura oblíqua que se estendia subgengivalmente na face vestibular. O trauma havia ocorrido há três anos e o paciente havia sido submetido a diversas intervenções endodônticas, apresentando no momento da consulta inicial por nossa equipe uma lesão periapical extensa. Após a realização da anamnese, exame clínico e radiográfico, foi adotada uma abordagem multidisciplinar conservadora com a manutenção do dente e a realização da enucleação da lesão periapical por meio de intervenção cirúrgica e posterior reabilitação. Em uma mesma sessão, foi realizada a obturação do conduto com cone de guta percha e cimento sealer 26 e a cirurgia parendodôntica juntamente com a apicoplastia. Uma semana após a intervenção cirúrgica foi realizada restauração classe IV com resina composta baseada no enceramento dos modelos de diagnóstico montados em articulador. Com base em uma avaliação criteriosa e multidisciplinar, foi possível adotar uma abordagem conservadora no caso em questão, com a manutenção e reabilitação de um dente permanente em um paciente jovem, evitando a indicação de exodontia(AU)


Cases of oblique dental fractures often have an unfavorable prognosis, usually with the indication of extraction of the dental element. The present article aimed to report the treatment of the clinical case of a 12-year-old male patient, who attended the Oral Health Unit of the Hospital Universitário de Brasília with a referral for extraction of tooth 7, presenting an oblique fracture that extended subgingivally on the labial surface. The trauma had occurred three years ago and the patient had undergone several endodontic interventions, presenting an extensive periapical lesion at the time of the initial consultation by our team. After anamnesis, clinical and radiographic examination, a conservative multidisciplinar approach was adopted with the maintenance of the tooth and the enucleation of the periapical lesion through surgical intervention and subsequente rehabilitation. In the same session, the conduit was filled with gutta-percha cone and sealer cement 26 and parendodontic surgery was performed together with apicoplasty. One week after the surgical intervention, class IV restoration was performed with composite resin based on the waxing of the diagnostic models mounted on an articulator. Based on a careful and multidisciplinary evaluation, it was possible to adopt a conservative approach in the case in question, with the maintenance and rehabilitation of a permanent tooth in a young patient, avoiding the indication of extraction(AU)

ABSTRACT

Objetivo: Describir el tratamiento de un paciente que pre- sentó un incisivo superior permanente con exposición pulpar y desarrollo radicular incompleto utilizando Biodentine como material para la protección pulpar directa. Caso clínico: Un paciente de 9 años fue derivado para la evaluación y el tratamiento del diente 11. El examen clínico y radiográfico reveló la presencia de una fractura amelodentinaria, con exposición pulpar y desarrollo radicular incompleto. Luego de lavar y desinfectar el área de la fractura, se protegió la pulpa expuesta con Biodentine y se restauró la cavidad. El paciente fue citado para control a los 18 y 25 meses. En los controles se comprobó que el diente se mantuvo asintomático y funcional. Luego de 25 meses, el examen clínico y radiográfico confirmó la presencia de pulpa vital y el completo desarrollo del remanente radicular. Los resultados obtenidos en el presente caso clínico sugieren que el empleo de Biodentine puede ser considerado como una valiosa alternativa para la protección de la pulpa ex- puesta en dientes con desarrollo radicular incompleto (AU)


Aim: To describe the treatment of a patient that presented a permanent maxillary central incisor with pulp exposure and incomplete root formation, using Biodoentine as the material for direct pulp protection. Clinical case A 9-year-old male patient was referred for evaluation and treatment of tooth 11. The clinical and radio-graphic examination revealed the presence of a dentinoe- namel fracture, with pulp exposure and incomplete root for- mation. After cleaning and disinfecting the area, the exposed pulp was protected with Biodentine and the cavity was re- stored. The patient was scheduled for control after 18 and 25 months. In these evaluations, it was confirmed that the tooth remained asymptomatic and functional. After 25 months, the clinical and radiographic examination showed the presence of vital pulp and a completed development of the root. The obtained results of this clinical case suggest that Biodentine can be considered a valuable alternative for pulp capping in teeth with incomplete root formation (AU)

We present a clinical case of root fracture of the middle third, piece 2.1 with displacement of the fracture ends. ABSTRACT

Objetivo: Informar el tratamiento de dos piezas con fractura radicular horizontal del tercio medio, patrones de curación y seguimiento por cinco años. Caso clínico: Se presentó a la consulta una niña de 9 años de edad con traumatismo de 10 días de evolución. Examen clínico: fractura amelodentinaria restaurada en la pieza 2.2, movilidad y sensibilidad a la percusión y palpación en ambos incisivos centrales superiores. Examen radiográfico: fractura radicular horizontal de tercio medio en piezas 1.1 y 2.1. Tratamiento: inmovilización con placa removible durante 4 semanas, indicaciones de higiene y uso, y controles de seguimiento. Al primero y al cuarto mes, ambos incisivos mostraron ausencia de movilidad, reacción positiva de sensibilidad y signos radiográficos de reabsorción superficial interna y externa. A los 6 meses, el 1.1 evidenció signos de reparación con tejido conectivo, y el 2.1, signos clínicos y radiográficos de necrosis pulpar del fragmento coronario. Se realizó el tratamiento endodóntico de la pieza 2.1 hasta el nivel de la fractura con pasta a base de hidróxido de calcio, y luego de la comprobación de la formación de una barrera de tejido duro, se obturó definitivamente con gutapercha y sellador endodóntico. Cinco años después del traumatismo, ambos incisivos se mostraron asintomáticos y los estudios por imágenes evidenciaron una completa consolidación de las fracturas. Conclusión: Un diagnóstico temprano, procedimientos apropiados de tratamiento, el conocimiento de los procesos curativos y un monitoreo cuidadoso de todos los parámetros clínicos y radiográficos son claves para un enfoque correcto y conservador de las piezas dentarias con fractura radicular (AU)


Aim: To report the treatment, healing patterns and fiveyear follow-up of two permanent incisors with horizontal root fracture located in the middle third. Case report: A 9-year-old girl who came to our consultation 10 days after a dental trauma. Clinical examination: restored enamel-dentin fracture in upper left lateral incisor; mobility and sensitivity to percussion and palpation in both upper central incisors. Radiographic examination: horizontal root fracture in the middle third of both upper central incisors. Treatment: stabilization with a removable splint for 4 weeks, indications for hygiene, use of the splint and follow-up controls. At first and fourth month, central incisors presented absence of mobility, positive response to pulp testing and radiographic signs of internal and external superficial resorption. At sixth month, signs of healing with connective tissue were found on the right central incisor, while the left one showed clinical and radiographic signs of necrosis of the coronal fragment. The root canal of this segment was treated initially with a calcium hydroxide paste and, after verifying the formation of a hard tissue barrier, it was filled with gutta- percha and endodontic sealer. Five years after the trauma, both central incisors were asymptomatic and imaging studies showed complete healing of the fractures. Conclusion: Early diagnosis, appropriate treatment procedures, knowledge of healing patterns and careful monitoring of clinical and radiographic parameters are key factors for a proper and conservative approach of injured tooth with root fracture (AU)

ABSTRACT

Objetivo: descrever uma reabilitação em incisivos superiores por meio da associação de coroas totais de cerâmica e retentor intrarradicular de fibra de vidro. Caso clínico: paciente P. C. G., gênero masculino, de 49 anos de idade, leucoderma, insatisfeito com a estética dos dentes anteriores devido a traumatismo dentário e fraturas recorrentes das restaurações. Após a realização de anamnese, exames clínico e radiográfico, contataram-se restaurações de resina composta insatisfatórias nos dentes anteriores, grandes diastemas entre os dentes e leve inflamação gengival. Para o plano de tratamento, optou-se por realização do tratamento endodôntico dos elementos 12 e 22, pino de fibra de vidro no dente 21, e coroas totais de dissilicato de lítio em todos os incisivos superiores. Considerações finais: com a finalização do tratamento e o retorno do paciente à clínica após seis meses, pode-se observar um tecido gengival sadio e o paciente muito satisfeito com os resultados obtidos. Objective: to describe a rehabilitation in upper incisors through the association of total ceramic crowns and intra-radicular fiberglass retainer. Clinical case: patient P. C. G. male sex, 49 years old, leucoderma, dissatisfied with the aesthetics of the anterior teeth due to dental traumatism and recurrent fractures of the restorations. After anamnesis, clinical and radiographic examination, unsatisfactory composite resin restorations on anterior teeth, large diastema between teeth and mild gingival inflammation were contacted. For the treatment plan, we chose to perform endodontic treatment of elements 12 and 22, glass fiber pin on tooth 21, and total crowns of lithium disilicate on all upper incisors. Final considerations: with the completion of the treatment and return of the patient to the clinic after six months, a healthy gingival tissue can be observed and the patient very satisfied with the results obtained. ABSTRACT

La extrusión dentaria o erupción forzada es una opción de tratamiento con enormes ventajas cuando nos vemos obligados a tratar fracturas corono-radiculares. Se define como el movimiento en dirección coronal a través de la aplicación de fuerzas ligeras y continuas para provocar cambios en los tejidos blandos y hueso. La extrusión dental forzada amplía las alternativas en cuanto a la rehabilitación futura del paciente, ya que se puede optar por la restauración periférica total con anclaje del resto radicular, o bien por la exodoncia posterior a la extrusión, con la consecuente mejoría de las condiciones del tejido óseo para una posterior rehabilitación implanto-asistida. Presentación de caso clínico, pieza dentaria 2.2 con fractura corono-radicular en la cual se realizó tratamiento de erupción forzada y rehabilitación protésica (AU)


Extrusion or forced eruption es a treatment option with enormous advantages when we are forced to treat crown-root fractures. It is defined as the movement in the coronal direction through the application of light and continuous forces to cause changes in the soft tissues and bone. Forced dental extrusion expands the alternatives regarding the future rehabilitation of the patient, since it is possible to choose either total peripheral restoration with anchoring of the radicular rest of the post-extrusion extraction, with the consequent improvement of bone tissue conditions for subsequent implant-assissted rehabilitation. Presentation of a case report: tooth 2.2 with a crown-root fracture in which forced eruption and prosthetic rehabilitation were performed (AU)

A metodologia e seus fundamentos são discutidos e um caso clínico é detalhado para melhor elucidação.


This article presents a special protocol for treatment of alveolar defects in molar regions where one ore two bone walls are absent without using Guided Bone Regeneration (GBR). Its methodological aspects and basic tenets are discussed in addition to a detailed clinica case for better comprehension of this interesting protocol.

ABSTRACT: Dento-alveolar traumas are one of the most frequent injuries to teeth, mainly affecting the upper incisors due to their exposed position in the dental arch. In such cases, esthetics, function and phonetics of anterior teeth may be compromised. Furthermore, when there is involvement of the biological width, there is often a poor prognosis. This case report describes the multidisciplinary approach to tooth fragment re-attachment in a fracture with biological width violation. The patient presented with an oblique crown fracture in the maxillary right lateral incisor, extending from the buccal to palatal side, as well as a biological width invasion. The re-establishment of the biological width was obtained by periodontal surgery to achieve clinical-crown lengthening and tooth fragment re-attachment with a glass fiber post to increase retention. After 3 years of follow-up, the rehabilitated lateral incisor remains in good condition, with satisfactory esthetic and periodontal health.

Abstract Crown-root fractures in primary teeth are rare, with a wide variation of patterns that make diagnosis and treatment difficult. The aim of this study was to present a case series of crown-root fractures in primary teeth of children who attended a reference center. The study followed 28 cases of crown-root fractures in 26 children, representing 4% of the total number of attendees over 11 years of reference service at the Universidade Federal de Pelotas, Pelotas, Brazil. In most cases, the maxillary central incisors (85.7%) were involved. In the crown, 57.1% of the cases had a simple line of fracture, and 42.9% had multiple lines. Simple lines predominated in the root (89.3%). In cases with simple fracture lines in the crown, 37.6% underwent a total extraction, 31.2% required a partial extraction and 31.2% underwent pulpectomy. In most teeth with multiple lines of fractures in the crown, total extraction was performed (91.7%) All cases were followed-up until the eruption of the permanent successor tooth, and all patients had favorable outcomes.


Resumo Fraturas coronorradiculares em dentes decíduos são raras, com uma ampla variação de padrões que dificultam o diagnóstico e tratamento. O objetivo deste estudo foi apresentar uma série de casos de fraturas coronorradiculares na dentição decídua de crianças atendidas em um centro de referência. O estudo acompanhou 28 casos de fratura coronorradicular em 26 crianças, representando 4% do número total de participantes ao longo de 11 anos no serviço de referência na Universidade Federal de Pelotas. Na maioria dos casos, o incisivo central superior (85,7%) foi o mais envolvido. Na coroa, 57,1% dos casos tiveram linha de fratura simples, e 42,9% tiveram múltiplas linhas. As linhas simples predominaram na raiz (89,3%). Nos casos com linhas de fratura simples na coroa, 37,6% foram submetidos à extração total, 31,2% necessitaram de extração parcial e 31,2% de pulpectomia. A maioria dos dentes com múltiplas linhas de fratura na coroa, a extração total foi realizada (91,7%). Todos os casos foram acompanhados até a erupção do dente permanente sucessor, e todos os pacientes tiveram resultados favoráveis.

Introdução: a odontologia prioriza a manutenção e integridade dos dentes na boca e a ocorrência de traumatismos dentais vem dificultando o cumprimento desta meta, uma vez que suas consequências envolvem danos funcionais, estéticos, psicológicos, sociais e terapêuticos, além dos altos custos para a reabilitação e acompanhamento por longos períodos de tempo. Objetivo: avaliar o conhecimento dos cirurgiões dentistas da região metropolitana de Juazeiro do Norte sobre as condutas clínicas baseadas nos planos de tratamentos propostos por estes profissionais sobre fraturas coronárias com ou sem exposição pulpar. Método: a partir de um questionário descritivo, foram abordadas questões referentes ao perfil dos profissionais entrevistados e procedimentos empregados na conduta frente a fraturas coronárias. Setenta e cinco questionários foram devidamente preenchidos, sendo 34 mulheres e 41 homens. Resultados: dos entrevistados 13 relataram nunca terem atendido casos de trauma dental, 26% relataram serem incapazes de tratar todos os casos de traumatismo dentário. E mesmo a maioria tendo formação especializada, grandes dificuldades foram encontradas nos planos de tratamento propostos em relação a fraturas mais complicadas. Discussão: as lesões dentais traumáticas envolvendo tecidos duros e de sustentação podem representar padrões de cicatrização bastante complexos. As lesões traumáticas do tipo fraturas coronárias apresentam um prognóstico favorável para a manutenção da vitalidade pulpar em relação às fraturas coronárias complicadas. As fraturas corono-radiculares com ou sem envolvimento pulpar, foram as que apresentaram mais indicações inadequadas de tratamento, provavelmente por necessitarem de uma abordagem multidisciplinar. Conclusão: considera-se primordial a multidisciplinaridade durante a formação acadêmica do clinico geral, bem como o aperfeiçoamento ao atendimento a pacientes com trauma dental. (AU)


Introduction: Dentistry prioritizes the maintenance and integrity of teeth in the mouth and the occurrence of dental trauma is hampering the achievement of this goal, since its consequences involve functional, aesthetic, psychological, social and therapeutic damage, in addition to high costs for rehabilitation and monitoring for extended periods of time. Objective: The objective of this study was to assess the knowledge of dentists in the metropolitan region of Juazeiro do Norte on the clinical management based on treatment plans proposed by these professionals on crown fractures with or without pulp exposure. Methods: For that, starting from a descriptive questionnaire, some questions regarding the profile of inquired professionals and procedures used in the performance of treatment of crown fractures. Results: Seventy-five questionnaires were fully completed, 34 women and 41 men. Only 13 reported ever having met cases of dental trauma, 26% reported being unable to deal with all cases of dental trauma. And even most having specialized training, great difficulties were encountered in the proposed treatment plans for the most complicated fractures. Discussion: Traumatic injuries involving pulp and periapical tissues can represent very complex healing patterns. The crown fractures have a favorable prognosis for the maintenance of pulpal vitality in relation to coronary complicated fractures. The corono-root fractures with or without pulp involvement, were the ones that had more inadequate treatment, probably because they require a multidisciplinary approach. Conclusion: it is essential for multidisciplinary academic training of general practitioner as well as improving service to patients with dental trauma. (AU)

El propósito de este reporte es presentar el caso de un paciente femenino con presencia de fractura radicular vertical (FRV) de un segundopremolar superior izquierdo, remitida a la clínica de Postgrado de Endodonciade la Universidad De La Salle Bajío para remoción de poste y retratamiento dental. Una vez realizado el diagnóstico se determinó FRV mediante sondeo minucioso irregular en un solo punto con sonda flexible, ligero dolor a la masticación y pérdida ósea radiográfica en forma de halo o gota, se sugirió como tratamiento extracción dental y cirugía exploratoria a petición del paciente


The aim of this report is to present the case of a female patient with a vertical root fracture (VRF) of a maxillary second premolar who was referred to the Postgraduate Endodontic Clinic of De La Salle Bajío University in León, Guanajuato, Mexico for the removal of a metallic post and retreatment of root canals. Examination via thorough irregular periodontal probing at a single point with a fl exible probe resulted in a diagnosis of VRF, slight pain on chewing, and radiographic bone loss in the shape of a halo or droplet, for which the suggested treatment was the extraction of the tooth, a surgical procedure that was subsequently performed at the patient’s request.

ABSTRACT

Los traumatismos dentarios constituyen uno de los motivos más frecuentes en la consulta odontopediátrica. Si bien las fracturas radiculares en dentición primaria son las lesiones traumáticas menos prevalentes (2-4 por ciento), son de difícil resolución y ocasionan la pérdida de dichas piezas en la mayoría de los casos. Objetivo: describir la resolución del traumatismo con fractura radicular en ambos incisivos centrales superiores primarios. Diagnóstico de la situación inicial: paciente masculino de 4 años que acude con su madre a la Cátedra de Odontología Integral Niños de la FOUBA presentando un traumatismo dental de 2 horas de evolución. Diagnóstico clínico-radiográfico: fractura radicular horizontal de tercio medio del 5.1 y 6.1 (sin reabsorción fisiológica radicular), sin desplazamiento del fragmento coronario, con laceración de labio superior y encía marginal. Secuencia de procedimientos operatorios: historia clínica con consentimiento informado; anamnesis médica y odontológica; examen clínico y radiográfico de 5.1 y 6.1 y antagonistas; limpieza de tejidos blandos con clorhexidina; impresión con alginato; confección de placa posicionadora para ferulización; prueba e instalación de la placa; indicaciones al paciente y a su madre: uso permanente durante 45 días y uso nocturno, para comer y hacer deportes 45 días más. Recomendaciones de higiene; controles inmediatos: 30 y 45 días y 3 y 6 meses. Resolución del caso: en todos los controles, ambas piezas se presentaron asintomáticas. A los 6 meses se observa la correcta cicatrización radicular y la presencia de vitalidad en ambos incisivos. ABSTRACT

Presentar un tratamiento preventivo que consiste en la realización de un ajuste oclusal y de una placa neuromiorrelajante, en un paciente con facetas de desgaste debidas a parafunción. Caso clínico: un paciente de 28 años de edad consultó por un control. Se observaron facetas de desgaste patológicas en el esmalte de la pieza dentaria 4.7, con una relación cúspide/fosa profunda, por lo que se decidió realizar una remodelación oclusal y confeccionar una placa de protección neuromiorrelajante, a fin de evitar una posible fractura de la pieza. ABSTRACT

Horizontal root fractures are a type of traumatic dental injury. Horizontal root fractures are classified as apical third, middle third, and cervical third of the root based on the fracture line and on the degree of dislocation of the coronal fragment. The prognosis of the tooth depends on the patient’s age, stage of root growth, mobility of the coronal fragment and diastasis of the fragments. Proper clinical and radiographic examination should be done to reveal apical root fractures. Hence, the treatment plan can be decided depending on the type of fracture and extent of fracture line. This review was conducted to evaluate the apical third horizontal root fractures in order to determine the appropriate therapy.

ABSTRACT

Objetivo: presentar la resolución clínica de una fractura dentaria incompleta mediante la realización de una restauración adherida de tipo onlay. Caso clínico: un paciente de 36 años de edad consultó por dolor al masticar enl a pieza 3.7, la cual presentaba una restauración plástica oclusal. Al removerla, se observó una fractura en dirección mesiodistal, sin movilidad de ambos cabos de fractura, por lo que se realizó una incrustación adherida de tipo onlay. Conclusión: el correcto diagnóstico de una fractura dentaria incompleta y la comprensión de su etiología son fundamentales para la elección de un tratamiento conservador y con un alto grado de predictibilidad, como lo son las restauraciones adheridas tipo onlay.

ABSTRACT

Objetivo: presentar el caso de un paciente con fractura dentaria de etiología parafuncional y su resolución implantoprotética. Caso clínico: una paciente de 65 años de edad, con fractura verticalc ompleta en la pieza dentaria 2.4, consultó por dolor al masticar. Se realizó la exodoncia atraumática del diente y se colocó un implante con corona provisoria, de forma inmediata. Finalmente, se colocaron un emergente y una corona libres de metal y se instaló una placa de protección neuromiorrelajante. Conclusión: la prevención y el diagnóstico tempranos de las fracturas dentarias atribuidas a parafunción, permiten la realización de tratamientos menos invasivos que conservan las piezas dentarias naturales del paciente

ABSTRACT

Radicular fractures in permanent teeth are uncommon injuries among dental traumas, being only 0.5-7% of the cases. Management of horizontal root fractures presents a formidable challenge for clinicians because of the difficulty of achieving a stable reunion of fracture fragments. This case report presents the management of horizontally fractured canine by approximation of fracture fragments, fiber splinting, and use of MTA - both as an obturating material and intra-radicular splint. Short-term follow-up of the case showed promising results both clinically and radiographically.
